Job summary An exciting opportunity has arisen for clinically focused, highly motivated clinical pharmacist IP that has completed their pathway who wish to expand their experience by working Onsite/Hybrid for a General Practice within the Salford PCN area. The correct candidate must have an IP qualification, have completed their CPPE Pathway and have at least 1 year experience working in a primary care setting. You will work as part of a multi-disciplinary team in a remote role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare. You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients. You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement. Person Specification
