Integrated Care Clinician (Nurse) The post holder will undertake telephone triage assessments of patients in an efficient way, advising on the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines. This role involves working autonomously, assessing and advising callers within agreed guidelines. You will have the opportunity to deliver outstanding patient care without the physical strains of hands-on care, in a non-patient facing setting. Main duties of the job

You will draw upon your clinical assessment skills to help and advise a broad spectrum of patients, from minor ailments and injuries to complex and urgent conditions. As a Band 6 Triage and Assessment nurse, you will have the autonomy to make decisions but will be supported by experienced senior clinicians and multi-disciplinary colleagues. Person specification

Qualifications

NMC Registered Nurse (adult or Child), or HCPC Registered Paramedic with relevant clinical practice experience. Evidence of undertaking regular update training and continuous personal/professional development. Experience

Strong listening and communication skills. Ability to work collaboratively in a team. Ability to work on own initiative and make decisions. Sound clinical decision-making capabilities. High levels of current clinical knowledge, computer literate, committed to personal development. Ability to speak Welsh is desirable. Aptitude and Abilities

Committed to ongoing training and development. Ability to work under own initiative without direct supervision. Flexible approach to work, including working nights and weekends.
