F ocussed, bringing together colleagues from medicine, nursing, pharmacy, social care, voluntary sector, therapy and community teams. Working in an MDT environment you will provide clinical input and leadership to the team as you collectively define a management plan which addresses the needs of the whole patient across physical, mental and social care needs. This new programme will require strong and innovative clinical leadership. The successful applicant will need to develop strong relationships with the teams involved in the MDT and wider system partners. This is an opportunity to shape an exciting new model of care in Furness. The Clinical Lead will also have responsibility for line managing clinicians working within the programme, ensuring high quality clinical care and participating in trust quality and safety work. Main Duties of the Job

Doctors within the CHIEF programme will provide leadership of MDT meetings, clinical reviews (either remotely or face to face via home visits or clinic appointments) and support to the wider team with senior decision making. Our data identified a very small number of patients ( The team will work during the working week with no evening or weekend work required, MDT meetings will be held throughout the week and are based in Barrow town centre. About Us

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre. FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay. Job Responsibilities

This is a novel programme at the early stages of development, we hope and expect that successful applicants will be part of shaping this exciting new service over the coming years. We invite applications from Consultants, GPs or experienced clinicians who are able to provide independent, senior decision making within the multi-disciplinary team. The programme is looking at adult patients only so we are looking for candidates with experience managing general medicine and frailty who understand the complexities of the community setting. Essential

MRCGP, MRCP or equivalent GMC Registration CCT in General Practice, General Medicine or other relevant speciality Experience

Essential

Previous experience managing undifferentiated acute medical illness in community or acute settings as a senior, independent clinician Experience of supervision of junior medical colleagues, Advanced Clinical Practitioners and/or specialist nurses. Effective experience in a leadership role Experience working as part of multidisciplinary team in any setting Desirable

Experience working as part of multidisciplinary team in the community Previous experience as Clinical Lead or GP Principle Experience in dealing with doctors in difficulty and managing disciplinary investigations Experience in managing budgets and developing QIPP schemes Knowledge

Competence interpreting clinical investigation results such as blood results, ECGs and medical imaging Ability to holistically manage adult patients presenting with acute and chronic health issues in a community setting. Ability to determine patient-centred practical management plans which optimise acute and chronic health conditions in concert with the wider MDT. Maintain up to date appraisal and revalidation.
