Job overview

We are looking for an enthusiastic and hard-working person to work across the Integrated Frailty Service in the Hospital at Home and Urgent Community Response within the Royal Surrey NHS Foundation Trust, based in Milford Hospital. Main duties of the job

You will work as part of a multidisciplinary team alongside therapists, doctors, Advanced Care Practitioners, nurses, and pharmacists to provide integrated and personalised care to the patients. We aim to provide hospital-based care in the home environment, avoiding unnecessary admissions to the acute hospital and supporting early discharges home. The post holder will be a key member of the Integrated Frailty Multi-professional team including Consultant Geriatricians, Advanced Clinical practitioners (ACPs), Specialist registered Nurses, Paramedics, Senior Physiotherapists, Community Rehabilitation Practitioners, and a GP. The Team will provide hospital-based care at home and an urgent response to patients who are experiencing a health crisis with the aim to keep them in their own homes safely and avoid a hospital admission. You will assess and treat referred patients into the Hospital at Home and Urgent Community Response team using evidence-based practice and clinical experience and specialist skills in this area. You will provide assessment and treatment to maintain patients in their own homes and stabilise those patients who have urgent care needs in the community and initiate services, which safely avoid unnecessary hospitalisation. Qualifications

HCPC registration BSc Physiotherapy/ Occupational Therapy (or above) Member of Chartered Society of Physiotherapy/ Royal College Occupational Therapy Member of special interest group Knowledge and Skills

Evidence to support knowledge base in a specialist area. Previous senior experience in specialty area Knowledge of systems used for data reporting and recording Employer certification / accreditation badges

You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
