Main duties of the job This role involves visiting people in their own homes to support and review their chronic condition disease. We are an integrated service so the role will include working as part of the district nurse team.
Skills and knowledge with chronic condition management will be beneficial.
We will support you to transfer your existing clinical knowledge and develop skills to become a successful Band 6 Integrated generic chronic conditions nurse.
Applicants will be required to participate in working other shifts as part of the role including late shifts, bank holidays and weekends.
The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.

Visiting patients in their own home to provide the chronic condition service within District Nursing.
The integrated District Nurse Team is based at Chepstow Community Hospital. We are an established friendly team with high standards of care.
As well as the District Nursing service we provide the rapid medical/frailty service.
Person Specification Skills Essential Team player/motivates/ability to work alone/ability to make informed decisions/good interpersonal skills
Experience/Skills Essential RN, evidence of ongoing professional development, Degree level Sufficient post registration experience Communication/clinical skills/indepth assessment skills/management of long term conditions/ability to work autonomously
Desirable Chronic conditions certificates, teaching experience Expanded clinical skills, knowledge of chronic conditions Flexible approach to the service
