The Integrated Neighbourhood AHP Team Liaison Practitioner will be responsible for acting as liaison between Specialist Community Teams to ensure representation of all Allied Health Professionals at the Active Case Management functions of the 5 neighbourhood teams. They will liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries. They will contribute to the facilitation of the planning, delivery, and evaluation of care pathways for a designated group of patients. Main Duties of the Job

The INT AHP Liaison Practitioner is responsible for ensuring the effective and efficient operation and link to specialist teams by developing an understanding of the referral criteria, referral and clinical pathways within each team. The role will be responsible for understanding the service pressures across AHP teams and the impacts on cases discussed. The post holder will contribute to complex case discussions offering clinical advice and opinions. As an experienced clinician, the post holder will be expected to challenge appropriately using specialist clinical skills and knowledge of clinical systems and processes. The post holder will be required to receive, audit, analyse and communicate a wide range of complex and sensitive information, including confidential service user/patient level information. The post holder will be responsible for relaying and interpreting clinical information relating directly to the patient's care and signpost appropriately based on clinical and social care information. The post holder will support in the development of person centred multi-disciplinary pathways and integrated care programmes with partners. About Us

Qualifications

Registered Allied Health professional - HCPC. Recognised teaching or mentorship qualification. Advanced assessment skills and navigation experience. Undergraduate degree / Master's degree or working towards. Knowledge and Training

Substantial post-registration experience in area of speciality. Experience in integrated MDT working. Proven evidence of improving the patient experience. Previous experience of research and audit. Current experience of line management of staff. Skills

Knowledge of current advances and pathways of care relevant to specialty. Ability to develop collaborative working relationships with multi-professional teams. Excellent communication skills and ability to negotiate and influence. Disclosure and Barring Service Check
