Integrated Urgent Crisis Team Occupational Therapist Part time- 30 hours - Permanent This is an exciting opportunity for a therapist to join the Integrated Urgent Crisis Team (IUCT). As a member of the Integrated Urgent Crisis Team, you will be expected to work as a senior clinician with a broad range of competencies who is able to adapt to a wide variety of clinical circumstances. You will be expected to be able to complete a holistic assessment for patients within the community setting. This post will allow the successful applicant to gain an outstanding skill set and help to provide a seamless patient journey across community areas. You will also be expected to work as part of the MDT to complete skilled rapid assessments for individuals referred into IUCT either from discharge from the hospital under the discharge to assess pathway or via the community for a rapid visit referred by family, public or other health/social care professionals including North West Ambulance Service and GPs. Main duties of the job

To work within an interdisciplinary Integrated Urgent Care Team (IUCT) managing a caseload of patients with complex needs using evidence-based, client-centred principles to assess, plan, implement and evaluate interventions. To be responsible for providing effective therapy assessment and intervention to patients in their own homes as part of the Interdisciplinary Team. To develop basic nursing and physiotherapy/occupational therapy skills to aid the rehabilitation process. To provide supervision and teaching to junior, untrained staff and students of all disciplines. About us

Education & Qualifications

BSc Honours / Occupational Therapy or equivalent HCPC Registered Demonstrate experience as band 5 Knowledge

High level communications skills Good interpersonal skills Show ability to work under pressure Good organisational and planning skills Knowledge of current legislation, policy and guidance in relation to Intermediate Care services, Experience of applying Clinical Governance process in Management Evidence of continuing professional development To be able to support audits and research within the team Experience

Experience in rehabilitation and working within a MDT team. Experience Intermediate Care Skills and abilities

Able to travel to various locations between Tameside and Glossop Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
