It is expected to be 12h shifts (8am-8pm), and we encourage positive work-life balance. To triage incoming referrals assessing the patient’s needs, determining the priority of care and processing the referrals accordingly. To contribute to the delivery of high standards of health care to patients by overseeing and responding to the remote monitoring systems supported by the senior clinical team.

To communicate regularly with patients and/or their carers, triaging clinical concerns and presentations as they arise and liaising with clinicians across the system, working in an integrated partnership.

To have awareness of the several integrated services demand and capacity across the system by developing knowledge and skills in SystmOne ledger management with the ability to highlight areas of escalation that could affect capacity.

This is predominantly a non-patient facing role but with regular opportunities to maintain clinical competencies.

Key Responsibilities:

To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary. To document triage notes in order that the patient’s record is complete and for audit purposes. To escalate concerns when appropriate by liaising with the clinical teams in the community and the integrated team. To assess, prescribe and arrange care interventions and assessments for all patients referred to the Single Point of Access [UCCH, 2h-Urgent Community Response (UCR), Rapid Response and Virtual Ward (VW)]. To work within the integrated team to prevent unnecessary admission to hospital utilising all available pathways.

This role will be part of Urgent Community Response, as a triage nurse for that cohort of patients, including Community Virtual Ward (CVW), both to triage and monitor patients remotely, liaising with our CVW teams.

You’d be joining initially one of the services, and building your skills to enable within all services, in time. The role will be based in a modern open-space office with a multi-organisational integrated team (Paramedics, GPS, nurses, social workers, support staff, call handlers).

Assess, plan, implement and evaluate all aspects of patient care, and develop care plans, using clinical reasoning skills, which may need to be delivered from a range of options.

Assess patients’ holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.

Liaise with GP’s, Social services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.

Have the ability to use the remote monitoring facility for Virtual Ward patients, understanding escalation points and when further clinical support is required.

Be accountable for work delegated to others within the team.

Ensure that Cleo, SystmOne (or other patient records used by the Service) inputting is completed on a daily basis.

Will be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies.

Additional Responsibilities:

To support clinical activity in Virtual Ward Hub and be competent in all aspects of that hub, interchanging role with the VW Triage Nurse when/if the activity increased in that service. To be responsible for clinical activity in UCCH and be competent in all aspects of that hub, within scope of practice. To be responsible for clinical activity passed by the NCHC SPoC as 2h-UCR and be competent in all aspects of UCR Triaging and allocation, within scope of practice.
