The post holder will work in close collaboration with other professionals, including the wider Multi-Disciplinary Team (nursing, psychiatry, psychology, and speech and language therapy) in assessing service users' needs, devising care plans, and evaluating outcomes that promote service user empowerment through the process. The post holder, with the wider Multi-Disciplinary Team, will provide high quality, individualized care, which addresses the needs of the service users and meets individual health needs.
The Intensive Support Team (ISS) will provide intensive support to adults with learning disabilities who are presenting challenging behaviour and/or mental health problems in the community to prevent, where possible, admission to hospital. The team will act as a gateway to the inpatient assessment and treatment service and work in close liaison with inpatient services to facilitate appropriate admission and timely discharge from services. The team will provide a 9am to 5pm service (there may be some variation to this) across the seven day week (including bank holidays) and will work with the service user in a variety of health and social care settings. The post holder may be required to work within 24-hour shift patterns to support the role in agreement with the Team Leader.
Main duties of the job
The post holder will:
Maintain responsibility for a caseload of people with learning disabilities and health needs, providing a holistic approach to service user assessment, treatment, review, and management of packages of care. Ensure that the statutory requirements of the Mental Capacity Act are observed and applied. Have the clinical skills relative to the delivery of care in the service area. Have effective working relationships with other professionals within the service area, across the Divisions, Trust, and other NHS providers and key stakeholders. Contribute towards effective management and utilization of resources within the service area. Provide clinical advice for service users who present challenging and mental health needs, that promotes and facilitates the positive health and wellbeing of people with learning disabilities, including direct work with service users, carers, colleagues, and other service providers.

KNOWLEDGE, TRAINING AND EXPERIENCE
Essential Academic qualifications related to the appropriate profession. Registered professional status. Membership of a professional body. Understanding of the health and wellbeing needs of people with a learning disability over a broad range of health issues.
Desirable Additional areas of training through short courses. ENB 998/Mentorship (Mentoring & Preceptorship) or equivalent. Postgraduate qualification. Teaching/training experience.
AREAS OF EXPERIENCE AND KNOWLEDGE
Essential Organizational skills and the ability to work effectively within a management structure. Communication skills - written, verbal, and non-verbal. IT skills, in particular MS Word. Experience of assessing and managing clinical risk. Experience in care planning.
