Interim Programme Manager , on a

£198.00 - £249.70 per Day Responsibilities: Manage LBB winter funding, deploying it to meet presenting needs ensuring increased capacity meets need, is delivered within the financial envelope and achieves maximum impact. Develop winter capacity and demand live dashboard to allow system monitoring and management. Identify system challenges, lead transformation and change. Develop social care pathways to reduce pressures on acute based care. Programme manage the roll out of re-directed acute funding, harnessing innovation and enhancing the current national best practice to gold standard model of care. Qualifications: Be educated to a minimum of first degree level or relevant equivalent work experience. Experience of working in complex multi-agency systems around discharge and admission avoidance. Knowledge of health and social care and the discharge system. Knowledge of national policy relating to hospital discharge. Please note: You should be available to work immediately or at short notice.
