We are looking for a Lead Nurse with experience or looking to develop clinical competencies in managing long-term conditions in general practice, specifically Hypertension (HTN), Chronic Kidney Disease (CKD), and respiratory disease (Asthma, COPD). This exciting role will hold responsibility for the delivery of long-term conditions (LTC) service at Haringey GP Federation. This role is open to a General Practice Nurse or a nurse from secondary care with relevant experience in managing patients with long-term conditions who is interested in progressing their career and leadership skills. Ideal Candidate

Ideally, the candidate will have: Experience caring for patients with long-term conditions and managing their own caseload of patients. Experience working in a team, supervising junior members of staff. Experience of EMIS Web or System One. Awareness of national guidelines and management of specific patient groups. ARTP Spirometry Accreditation or willingness to work towards accreditation. Experience working at an advanced level, caring for patients with long-term conditions. About us

Qualifications

NMC pin registration Minimum 5 years post-registration experience At least 3 years recent primary community or other relevant nursing experience Is a nurse prescriber Experience

Experience in nurse-led management of long-term conditions Experience of leading a Nursing team and/or health care support teams in a healthcare setting Experience in developing and implementing protocols and clinical guidelines Experience in quality initiatives such as clinical benchmarking Experience of leading a team in a multisite organisation Disclosure and Barring Service Check
