The main focus of the job role is to minimize unnecessary hospital admissions and ensure prompt discharges, in alignment with Trust Initiatives. This will require the post holder to establish links with the UECC, SDEC, AMU, and other teams and wards within the Acute Trust to establish the new integrated pathways. This role is pivotal in orchestrating a smooth and efficient patient care journey. The post holder will co-ordinate the patients' journey providing excellent care and develop consistent approaches to the strategic management of patients through training, education, mentorship, policy and guideline development, audit, research, and implementation of evidence-based practice. The post holder will use various methods of communication to challenge existing practice about clinical decision-making with senior Health Care Professionals to ensure patients are able to access the most appropriate clinical pathway. This will require the post holder to demonstrate excellent negotiating and influencing skills along with high-level clinical expertise and awareness about the wider community services. The post holder will have a Trust-wide responsibility to promote clinical excellence in the caring of patients presenting to their area by providing clinical service and support to nursing staff and other colleagues within the multi-disciplinary team. Main Duties of the Job

Establishing efficient cooperation with the Acute Trust staff during ward rounds and MDTs to discuss patient cases effectively. To be a pivotal member of the Team. Seeing patients in a variety of settings including (not limited to) new referrals to the specialty, undertaking telephone calls including telephone triage when required, and participating in ward rounds. Work as an autonomous practitioner. Managing and prioritizing own workload. Work autonomously and be effective and responsible for supporting others and exploring mechanisms to develop new ways of working. Showcasing strong clinical leadership, providing supervision and support to the nursing team to advance clinical excellence. Promoting open and effective communication about complex or challenging issues, overcoming any barriers to communication. About Us

Knowledge

Management and leadership knowledge gained through participation in relevant educational opportunities or through significant experiential learning. Knowledge and experience of providing Clinical Supervision. Qualifications

Educated to first degree level or demonstrable equivalent experience. Relevant Training and/or a Qualification in Management. Post-registration education relating to the Care of Older People and/or the Management of Long-Term Conditions. Nurse Prescribing Qualification. Experience

Substantial post-registration experience gained in areas involved in the care of older people and those with long-term conditions. Significant experience of staff management working at band 6 level or above. Experience of working in or closely with primary and community services. Experience of incident and complaint investigation including the development of comprehensive reports and action plans. Recruitment and selection experience.
