The role of the Health Facilitator will be to improve and facilitate access to care in primary services, through supporting the GP practices and community services with health issues and the needs of people with learning disabilities. The post holder will also cover some work as an acute liaison nurse to allow smooth transition to and from hospital services. To provide specialist learning disability knowledge, expertise and input to support the work of the access to healthcare for people with learning disabilities, to ensure that people with learning disabilities living in West Sussex can effectively access primary and secondary health care services as required, resulting in improved health outcomes. To support implementation of the Directed Enhanced Service (DES) Annual Health checks for people with Learning Disabilities, through provision of specialist education and support to Primary Care Staff to ensure that practices are able to meet its requirements and to promote uptake of the DES amongst practices across West Sussex. To identify training needs for local services and people with learning disabilities and their carers and deliver as appropriate in conjunction with people with learning disabilities. Qualifications
Registered Learning Disability Nurse Qualification (RNLD / RNMH) or Allied Health Profession with Learning disabilities experience. Evidence of on-going training in specialist area and commitment to development. Full driving licence or ability to move around West Sussex for appointments and meetings. Experience
Working with people with learning disabilities and their carers Developing and using easy read health education materials Experience of working collaboratively and in partnership with a range of health and social care professionals and users and carers to plan, develop and evaluate services Experience of developing Health Action Plans and undertaking a health facilitation role.
