Job summary We have a vacancy for a Band 6 learning disability nurse in our health facilitation team, the main function of which is to work closely with adults with learning disabilities to ensure they get their health needs met. If you are an enthusiastic and motivated learning disabilities nurse, keen to work as part of a much wider MDT, we would love to hear from you. The work is varied and can take place in different settings, depending what works best for the person involved. We support people with a variety of health issues, including physical health and access to primary or secondary care, emotional wellbeing or mental health concerns. There is an expectation that you can work independently to support the person with learning disabilities to get their health needs met. However you will often also be liaising closely with other health colleagues, both within our MDT or from primary or secondary care, social care staff, education, paid providers as well as with family carers, so excellent communication skills are required. This role will be based in the health facilitation team, working from the main office at Waddiloves in Bradford but you will need to be able to travel & do home visits etc across the whole of the Bradford district. We would happily interview nurses from other branches who have experience of and enthusiasm for working with & supporting people with learning disabilities to get their health needs met. Main duties of the job The post holder will work collaboratively with people with learning disabilities, their carers, professionals and agencies to provide a high quality, comprehensive service to assess and plan health care , with the central aim of maintaining and improving the health and wellbeing of adults with learning disabilities. This will include but is not limited to, working directly with service users who have complex healthcare needs, care coordination, supporting others to act as Health Facilitators, supporting health providers to make reasonable adjustments to care pathways and engaging in a range of health promotion activities. The role is advertised for learning disability nurses - however we would be open to interviewing nurses from other settings including adult, child or mental health if they have skills and experience relevant to the role. We would also be willing to consider Band 5 nurses who may feel they don't currently have the skills or experiences to be a Band 6 but would be willing to take on a development role to work towards this.
