An exciting opportunity has arisen to join our team at LNWH to support the Lead Nurses for Learning Disability & Autism and the Named Professional for Safeguarding Adults in the delivery of care for patients who have learning disabilities and/or autism. The role is responsible for maintaining the Trust's strategies alongside the Learning Disability & Autism and the Named Professional for Safeguarding Adults. The post holder will facilitate and support open and easy access to general hospital services for people with learning disabilities, autism or both from admission to discharge. Be the interface between mainstream general hospital services, primary care services and community specialist learning disability and autism services. Main Duties of the Job

To act as a point of contact for all LNWH NHS Trust staff, community learning disability (LD) services, social services and other professionals where the Lead Nurses for Learning Disability & Autism are not available. To improve the quality of care received by people with learning disabilities/Autism in hospital and to reduce the incidents of poor practice reported. Actively assist people with learning disabilities, as well as carers and health and social staff when using acute services. To facilitate open and easy access to general hospital services for people with learning disabilities/autism in the area. Seek appropriate support in regards to best interests and consent to treatment. Assist in the delivery of Learning Disability and Autism Awareness training sessions. Engage with meetings with nominated 'learning disability champions'; providing updates and resource materials as required. Promote the use of "Health Passport/ Personal Health Information File" for LD patients prior to or on admission in conjunction with the Community LD Nurse, Patients and families/ carer/s. Raise the profile of the health care needs of people with a learning disability across secondary care provision. About Us

Registered Learning Disability Nurse or Equivalent Health and Social Care equivalent professional qualification. At least 2 years Experience of working with Children and young people and adults with Learning Disability and Autism. IT Skills Proficiency, use of electronic patients records and data analysis. Being Proactive and ability to coordinate effective care and work closely with Multidisciplinary teams. Disclosure and Barring Service Check
