Job Description Service Care Solutions has a new and exciting opportunity which has become available working as a Leaving Care Personal Advisor with Cheshire East Council. Cheshire East Council is currently looking for someone who is happy to work 37 hours per week. Job Purpose As a Personal Advisor, you'll act as a corporate parent for young people preparing to leave the care of the local authority. You will provide qualitative assessments, ongoing support, and guidance to empower care leavers in developing the necessary skills for independent living. This role aims to support care leavers in realizing their full potential, contributing to their emotional, social, and financial independence. Key Responsibilities Relationship Building:

Establish and maintain meaningful relationships with care leavers, ensuring their views are heard and their needs are central to all support efforts. Pathway Planning:

Participate in the assessment, preparation, and review of pathway plans to equip young people with essential life skills for independence. Service Coordination:

Facilitate access to relevant services, working collaboratively with partner agencies to support the progress and well-being of care leavers. Documentation:

Maintain comprehensive, up-to-date records of interactions and services provided using the Council's electronic systems. Crisis Response:

Provide flexible, rapid response support in crisis situations, which may include work outside of typical office hours. Equal Opportunities:

Promote the best interests of care leavers, advocate for their rights, and take necessary measures to ensure their well-being. Requirements Essential Qualifications:

NVQ Level 3 in Advice and Guidance, Health and Social Care, or equivalent education. Knowledge & Skills:

Understanding of young people's issues, assessment methods, and education/employment trends. Ability to work effectively within a team and build partnerships across diverse groups. Proficient in IT applications and effective time management. Personal Competencies:

Excellent communication skills, flexibility, adaptability, and commitment to equal opportunities. Other Requirements:
