Participate in the assessment, preparation, and review of pathway plans to equip young people with essential life skills for independence. Service Coordination:
Facilitate access to relevant services, working collaboratively with partner agencies to support the progress and well-being of care leavers. Documentation:
Maintain comprehensive, up-to-date records of interactions and services provided using the Council's electronic systems. Crisis Response:
Provide flexible, rapid response support in crisis situations, which may include work outside of typical office hours. Equal Opportunities:

Understanding of young people's issues, assessment methods, and education/employment trends. Ability to work effectively within a team and build partnerships across diverse groups. Proficient in IT applications and effective time management. Personal Competencies:
Excellent communication skills, flexibility, adaptability, and commitment to equal opportunities. Other Requirements:
