Leaving Care Personal Advisor Service Care Solutions are currently recruiting for a Leaving Care Personal Advisor in Bedfordshire. The Leaving Care Personal Advisor will provide essential support, advice, and guidance to care-experienced young people as they transition out of the care system. Pay rate - £14.99 PAYE / £18.46 Umbrella Main Responsibilities As a Leaving Care Personal Advisor, you will be responsible for: Managing a caseload of young people leaving care, ensuring that Pathway Plans are developed, monitored, and reviewed regularly in partnership with the service user. Completing Pathway Assessments in a timely and collaborative manner, ensuring active participation from the young person, carers, social workers, and other professionals. Providing consistent and ongoing support to care leavers, assisting them in accessing resources and services, and offering advice and guidance on a range of issues that may affect their transition into independent living. Working in partnership with other professionals, including social workers and carers, to ensure a comprehensive and coordinated approach to the young person's support.
