1 x Full-time (37.5 hrs per week) Band 5 Newly Qualified Social Worker (NQSW) vacancy in the Older Person and Physical Disability Adult Social Care Community Team Lichfield We have an exciting opportunity for a band 5 newly qualified social worker (NQSW) to join the Lichfield community team. If you are due to qualify as a social worker in 2024/25 then we would encourage your application. You will be part of a dynamic, responsive team supporting people in the community with long-term health and social care needs. This role would suit someone who is dynamic, resourceful, confident in evaluating risk with excellent prioritisation, time management and keyboard skills. You will be fully supported by your Practice Lead and as an NQSW you will receive regular fortnightly supervision. The role will involve assessing people via phone, video link and in person dependent upon the person's wishes and situation. We work with adults over the age of 18 with a physical disability and older adults over 65 living with either physical disabilities and/or a mental health dementia diagnosis. Main duties of the job

The successful candidate will have a flexible approach and be a team player who is able to forge excellent working relationships with multi-disciplinary teams and partner agencies. The applicant will work within a range of relevant principles including Recovery and Personalisation; empowering individuals to take control through personalised, strengths and asset-based models of assessment and intervention and putting them at the centre of everything we do, recognising the rights of all individuals to gain access to valued roles, relationships, activities and facilities within their local community. You will need to be a registered Social Worker and will have responsibility for completing statutory assessments under the Care Act 2014 to facilitate safe and timely discharge from D2A pathways within the community hospital setting. The role will involve assessing; accessing; and implementing a range of social care services, and providing guidance/advice and information for adults with acute or long-term physical health, mental health needs and for older people and carers. The post holder will be required to undertake innovative strength-based assessments, care planning and review in line with the Care Act, 2014. There will be a requirement to undertake case management of complex casework and develop positive working relationships with service-users, carers, their support networks, the community, and other professionals. The role will provide opportunities to be part of section 42 safeguarding enquiries. The appropriate level of support and training will be provided to support the post holders professional development and gain confidence in this area. This is an interesting and challenging post for someone who is passionate to enhance their social work learning and has a desire and commitment for outcome-focused work which will bring about positive change to the people we support. This is a full-time position (37.5hrs per week). MPFT are committed to offering blended working options with a mix of home and office-based working. Qualifications and Training

Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body Ensuring current registration with the Professional Social Work regulatory body at all times A commitment to ongoing personal and professional development To undergo appropriate training to achieve Assessed Year in Practice (ASYE) Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint A clear understanding of and commitment to equal opportunities and anti-discriminatory practice Experience

Experience of working with Adults and older Adults with mental health needs and their carers Experience of social work in other user groups/settings i.e. Children and Families, Learning Disability, Mental Health. Disclosure and Barring Service Check
