You will be advocating and working with adults with complex needs and their carers, working collaboratively with colleagues from other agencies to develop support and services appropriate to people's individual needs utilising a strength based approach. The remit of the service is to support the citizens of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and/or mental health needs. The post holder will need to be confident carrying out outcome-focused assessments, with support planning, to meet their care and support needs. The post holder must be able to work with a diverse group of individuals and with our partner agencies to deliver the best outcomes for our citizens. The Trust is committed to safeguarding and promoting the welfare of children and young people/vulnerable adults and expects all staff to share this commitment.
