Liverpool Carers Centre was established by Local Solutions to deliver a range of services that improve the quality of life for carers in Liverpool. As well as undertaking carers assessments, the Carers Centre also offers a flexible support service that provides impartial advice & information. They can also signpost carers to specialist organizations depending on individual needs and circumstances. Job Summary
As a member of Liverpool Carers Centre team, you will work with unpaid carers to support them to access practical and emotional support. This includes supporting carers through the Carers Assessment process, which will be achieved in a variety of ways, such as visiting carers in their own homes or elsewhere to offer support. The role is based predominantly in the community with home visits being the primary method of contact. Key Responsibilities
Completing Carers Support Plans with carers using a person-centred approach. Providing Carers with information about the range of services available for carers and how to access them. Completing and submitting carers support plans in a timely manner to ensure departmental KPIs are achieved. Liaising with colleagues in statutory services. Being an energetic and solution-focused worker who will support carers to access the most appropriate services to assist them in their caring role. Developing relationships throughout the city that help and support unpaid carers while being a main enabler for holistic solutions. Contact Details:
