RATE: up to £26.36 ph (umbrella) TYPE: LOCUM HOURS: This is a full time role but flexible hours will apply and is hybrid TEAM: Adults Support Team Job profile: To deliver excellent support to vulnerable adults with a Learning Disability have a physical disability, Autism, Challenging Behaviour, Mental Health, Dementia. Work in partnership with Services/schools/colleges on our transition groups for Resident’s who are leaving children, and young people’s provisions and moving into adults’ provisions. Assisting Residents in identifying progression opportunities and providing on-going transition support for those moving into college, employment, apprenticeships or further learning. Being able to live the life they want. Establishing positive relationships with our Residents, supporting and enabling them to increase their confidence and realise their potential, fully integrating them into all project activities, and to explore services e.g Independent Living establishments as required, and a Curriculum for Independent Living skills.
