Job Summary: Under the supervision of the Team/Program Manager, the HH clinician is accountable for effectively managing or caring for a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate. This position coordinates the interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. These responsibilities will be carried out in accordance with organization policies/procedure and standards consistent with Federal, State and accreditation regulations. Requirements: Equivalent education required for RI LPN licensure; Bachelor’s Degree (min) in relevant discipline strongly preferred. Valid RI LPN license required. A minimum of one (1) year progressive clinical experience required. Previous experience in an acute care or home health care environment preferred. Valid CPR certification required. Valid driver’s license and automobile liability insurance are required. Ability to read and interpret documents such as medical records and medical reports. Ability to write reports, business correspondence, and procedure manuals. Must be able to speak, read, and write English with excellent oral and written communication skills required. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram, or schedule form. Must be able to deal with several abstract and concrete variables.
