Schedule: Every Other Saturday & Sunday 9am-11pm

Our Residential Program is looking for compassionate LPN professionals to work with medically involved individuals in a 24-hour supported home environment located in Swansea and surrounding communities. The LPN will provide premier services to the individuals in our care.
Responsibilities:
Provide a structured routine that includes house meetings, individual outings, meal preparation, ADL care, facilitate family visitations, and distribute medication while maintaining human rights. Implement Positive Behavior Support and Individual programs as identified in the Individual Service Plans. Utilize de-escalation and crisis management techniques as required to maintain safety. Requirements:
MA state valid LPN license. Qualifying criminal background check and a valid driver’s license. TB screening and Physical required upon hire and a TB update every two years.
