Job summary Provide clinical support, expert care and clinical leadership to support patients with a diagnosis of colorectal cancer at ASPHTo provide expert, comprehensive knowledge and specialist advice, information and psychological support to patients with a diagnosis of a colorectal cancerSupport the faster diagnosis and improved outcomes for patients with a suspicion of colorectal cancerPositively impact the experience of patients and carers by providing high standards of clinical practice and care as appropriate across the trust.Deliver specialist nurse led clinics for surveillance and treatment plans post MDT, and oncology clinics. Job description Job responsibilities To plan, deliver and evaluate programmes to address patients complex health needs.Initiate and promote collaborative working relationships and effective communication between all members of the MDT to improve the management and outcomes of patients within the specialist areaDiscuss investigations and treatment for cancer with sensitivity, knowledge and expertise.Deliver clinical care and management in line with local and national guidance and best practiceTo plan, deliver and evaluate programmes to address patients complex health needs Person Specification Qualifications Essential oFirst level degree in related subject or equivalent knowledge, skills and competency gained through clinical experience working at a senior level oPost-basic qualification in cancer or a relevant subject in colorectal cancer care / management or relevant experience in colorectal cancer nursing. oEvidence of continued professional development Desirable oAdvanced communications skills training or prepared to work towards oSACT certification Experience Essential oRelevant post registration experience at Band 6 or above, in cancer nursing. oEvidence of advanced communications skills oAbility to lead and influence change Desirable oPrevious Band 7 experience Knowledge Essential oAdvanced knowledge of colorectal cancer treatment and management oKnowledge of cancer performance and standards Desirable oAwareness of annual Cancer Patient Satisfaction Survey Skills Essential oEffective communication with all service users and with other staff to ensure that care, treatment and support is not compromised oAbility to hold a clear strategic vision and develop implementation plans to contribute to the development of the service.
