The ideal candidate will be a Pharmacist with significant clinical experience and being an independent prescriber is desirable. The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long-term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription requests, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN. Qualifications
GPhC registered pharmacist. Flexibility to work outside of core office hours. Disclosure Barring Service (DBS). Evidence of continuing professional development. Hold a GPhC independent prescribing qualification. Minor ailments certification. Membership of the Royal Pharmaceutical Society. Experience
Minimum of two years working as a pharmacist demonstrated within a practice portfolio. An appreciation of the nature of GPs and general practice. Experience and awareness of common acute and chronic conditions that are likely to be seen in general practice. In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare. Understanding the mentorship process.
