The role includes home visiting; providing medicines management interventions to patients within their own home, involving lone working without direct supervision. You will liaise with a wide range of Health & Social Care professionals to identify solutions for patients in a multidisciplinary network with a focus on promoting independence, prevention, and early intervention.
