Job overview

We are looking for an enthusiastic pharmacist to join our medicines optimisation team on a permanent basis. Working within Hertfordshire and West Essex Integrated Care Board and supporting primary care across the area, you will work with the team of medicines optimisation health care professionals to ensure appropriate medicines use across Hertfordshire and West Essex. You will respond to requests for medicines commissioning advice, perform medicines usage reviews, implement prescribing changes, undertake audits and improve systems and processes. There will be the opportunity to develop local guidance and undertake formulary reviews in a supportive environment. The role includes the opportunity to really make a difference by implementing prescribing strategies and liaising with GP practice teams, Primary Care Network Teams, Integrated Neighbourhood Teams, Community Pharmacy Teams, Secondary Care, and other health care practitioners. Main duties of the job

The candidate must be a member of the General Pharmaceutical Council. They must demonstrate sound clinical knowledge and the ability to communicate effectively. The right personal skills are crucial to success in this post. This is a learning environment where training and personal development are actively supported. Good computer literacy, the ability to multi-task, great team working and experience within primary care are desirable attributes. Herts and West Essex ICB supports home working, although the candidate will also be required to travel to meet the needs of the role. Full Time and Part Time hours considered. Qualification

Member of the General Pharmaceutical Council (GPhC) Pharmacy Master's Degree or equivalent Evidence

Evidence of continued professional development Evidence of analysing patient records for medication review and making judgements about appropriate recommendations, even where information is lacking, or medical (or other) opinion differs Experience of managing risks and reporting Ability to analyse complex issues where material is conflicting and drawn from multiple sources The promotion of equality of opportunity and good working relations Experience of working in primary care or a commissioning environment Knowledge

Good working knowledge of national & local pharmaceutical and prescribing policies, legislation and guidance which relate to medicines supply, prescribing and GP/community pharmacy practice Skills

Demonstrated capabilities to manage workload, working to tight and often changing timescales Experience of creating and giving presentations to a varied group of stakeholders Experience of communicating often complex medicines-related information to key stakeholders and bringing about a change in practice Employer certification / accreditation badges
