Main duties of the job The main duties include but are not limited to:
Undertake consultations with patients taking a comprehensive patient history, carry out physical examinations, and use their expert knowledge and clinical judgement to identify potential diagnoses.
Exercise a higher degree of personal and professional autonomy using advanced clinical practice skills to include clinical interventions.
Refer patients for investigations when necessary/appropriate.
Make a final diagnosis, decide on and carry out treatment, including the prescribing of medicine or referral to primary or secondary care specialist services within local agreed pathways and protocols as appropriate.
Ensure that all treatments and care provided are based on best practice/evidence-based care.
Plan and provide skilled competent care to meet the patients' health and social care needs, involving other members of the healthcare team as appropriate.
Work with multi-disciplinary and multi-agency teams in order to ensure patient needs are met.
Assess and evaluate, with the patient, the effectiveness of the treatment and care provided and make changes as needed.
Use independent/supplementary prescribing skills for the benefit of patient care.
Work independently and as part of the Urgent Same Day GP and ANP/ACP Hub Team.
About us We are a newly formed Urgent care hub for the patients of Boyatt Wood Surgery and Eastleigh Medical Practice. Eastleigh Medical Practice is a busy, town centre practice in Eastleigh. Boyatt Wood Surgery is a partner practice about a mile from central Eastleigh. Both practices are part of Eastleigh PCN. Eastleigh Medical Practice currently has over 14,700 registered patients and Boyatt Wood has 8,500 patients. There is excellent support from a multi-disciplinary team of GPs, ANPs, and Practice Nurses. The computer system is EMIS web.
Job responsibilities The post holder will demonstrate courageous and critical thinking and is a caring, compassionate, and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
Person Specification Experience
Experience of working within a nursing team in a mental health or emergency care setting.
Experience of dealing with vulnerable patients.
Experience in acute mental health setting, A&E.
Qualifications
Registered Mental Health Nurse.
Non-medical prescribing.
Evidence of continuing education.
Possession of 1st degree in Mental Health Nursing.
MSC in Advanced Practice or working towards.
Time management and able to prioritise.
Evidence of recent teaching, mentorship, and supervision of staff.
Evidence of recent audit.
Evidence of multi-professional working.
