These include Outreach support, Accommodation support, and wellbeing opportunities in one of our three Resource Centres.We are focused upon recovery and work to facilitate an individual’s transition from social isolation, crisis, or hospital, to ensure that they have the best opportunity and the required skills to maintain an independent life.If successful, you will manage a modest team that engages with individuals who have complex needs. You will contribute to a multidisciplinary approach that includes both health and social care professionals.You will need to demonstrate excellent communication skills (both written and verbal), be able to navigate ICT systems with confidence. You will need a high level of motivation and organisation, to think laterally.We are looking for someone who has an extensive experience in a providing or supervising role in Social Care. You will have: NVQ level (or higher) in care or working towards this level, to be achieved within months in post (Essential)Experience of delivering frontline care including mental health supportA minimum of -year supervisory experience in a health or social care environmentUnderstand the concept of Recovery and its application in order to develop an environment and service that enhances recovery (Essential)A knowledge of policies and procedures relating to social careExperience of undertaking reviews and complex risk assessmentsAble to evidence a person-centred approachMotivate, train and enable colleagues in order to deliver a high-quality service.Excellent communication both verbal and written and an ability to liaise and work with other professionals and agencies as and when requiredBe able to work on own initiative, taking decisions where appropriate and reprioritise tasks when required.
