In Bolton, the planned model of service delivery is for health and social care services to integrate, which in turn will deliver better quality outcomes for people. As a Mental Health Practitioner, you will provide the mental health expertise within the integrated neighbourhood team, working to support, advise, consult, and, where appropriate, care for individuals presenting with mental health-related problems. Main duties of the job

Work as a member of Integrated Neighbourhood Teams. Maintain up-to-date evidence-based knowledge of both functional and organic mental health disorders and the treatments/services available. Understand the needs of service users across all ages. Provide skilled mental health advice to service users, carers, and other professionals in the community regarding both functional and organic mental health disorders. Collaborate with Adult and Older People’s mental health services as appropriate and work in partnership with health and social care services. Maintain a close working relationship with local teams to step up or signpost as clinically indicated. Provide and coordinate formal and informal training to GP’s, primary care, and social teams on topics relating to mental health as agreed. Person specification
