Responsibilities: Conduct comprehensive mental health assessments and develop personalized treatment plans for patients. Direct to appropriate services to provide evidence-based interventions and therapy to individuals, couples, and families dealing with various mental health issues. Collaborate with the general practice team, including doctors, nurses, and allied health professionals, to ensure integrated care for patients. Offer primary care level of crisis intervention and support to patients in need of immediate assistance. Educate patients and their families about mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date clinical documentation and patient records. Participate in case conferences and multidisciplinary team meetings to discuss patient progress and coordinate care.
