The Older Adult Community Mental Health Team (OACMHT) is seeking a highly motivated practitioner who is passionate about advocating for Older Adults. The practitioner should be adaptable, creative, and possess the ability to work autonomously as well as part of a multi-disciplinary team. The team provides intervention-based care to service users with complex mental health needs in the Preston Central, Chorley, and South Ribble area. The successful applicant will work with service users who have severe and enduring mental health issues, significant functional/psychological mental health needs, dementia, and comorbid physical health issues. We collaborate with families, carers, and other agencies to promote well-being and deliver evidence-based interventions. Applicants should be open to new ways of working, foster recovery-based care, and engage in community transformation. Competency and experience in line with the person specification are required, along with a willingness to develop skills. Lancashire & South Cumbria NHS Foundation Trust offers a range of services, including secondary mental health care, inpatient child and adolescent mental health services, perinatal mental health, and forensic services. The Trust also provides physical health and well-being services in the community in collaboration with partners in Lancashire, Cumbria, and Sefton.
