Job overview Mid CRHT are currently looking to recruit Band 6 practitioners to join the team.

CRHT operates on a 24 hour, 7 day a week basis. CRHT offers to provide intensive home support to services users as an alternative to hospital admission. CRHT work collaboratively with a variety of services to support service users at home.

The post holder will be expected to work on a rotational shift basis in accordance with service demands, this includes working weekends and night shifts.

Main duties of the job The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care within the framework of Effective Care Co-ordination (ECC).

The post holder will operate as part of an integrated Home Treatment Service but be expected to be flexible in terms of delivering care within a variety of environments.

The core roles include; face to face assessment, specialist mental health assessments, supporting and sharing information with the wider police community and acting in an advisory capacity for people in conjunction with mental health law.

Detailed job description and main responsibilities

Formulate, contribute to, and participate in, the development and implementation of assessment and care planning systems in collaboration with the multidisciplinary team.

Manage a case load within the Crisis Resolution /Home Treatment Team.

Carry out generic and specialist assessments, including risk and provide therapeutic interventions/home treatments.

Ensure record keeping is in accordance with Professional and Trust standards.

Work in a professional and collaborative manner with the multi-disciplinary team.

Participate and promote positive intra-agency working.

Act as knowledge resource in specialist area of practice to other team members and other agencies as required.

Person specification Qualifications

Registered Mental Health Nurse or Dip.SW/CQSW, Dip.COT/SROT

Evidence of post registration study / continuing professional development

Approved training in phlebotomy

Knowledge/Experience

Experience of Mental Health assessments

Knowledge of developments in community care and of mental health legislation.

Experience of Crisis Intervention

Skills

Able to communicate effectively both verbally and non-verbally

Possess excellent listening and negotiation skills

Ability to work as a member of a multidisciplinary team
