Mental Health Rehabilitation Flow Practitioner

An exciting opportunity has arisen for a Band 6 Mental Health Nurse/LD Nurse, Occupational Therapist, or Social Worker to join the Rehabilitation Flow Team on a full-time, permanent basis.

This is a challenging, varied role, where you will use your clinical, operational, and organisational expertise to support Lancashire Service Users on their rehabilitation journey. You will complete assessments/reviews and ensure that the services are safe and meeting agreed service user outcomes in the provider placements.

The right candidate will have a desire to optimise the service user's rehabilitation experience whilst supporting and improving flow within the rehabilitation pathway, working closely with multi-disciplinary colleagues. There is a requirement for clinical experience in acute mental health settings with a desire to improve the patient experience and journey.

The successful candidate will be required to work independently using initiative, applying a high degree of confidentiality to all of their work. The right candidate must demonstrate good interpersonal and communication skills, manage competing demands, and organise their own work to contribute to the smooth operation of the Rehabilitation Flow Team.
