Mental Health Support Worker

The Role: Providing support to customers living in the community who have severe and enduring mental health diagnoses Working closely with specialist providers to deliver support to customers, helping them to further independence, access local services, and enjoy being part of their local community Providing appropriate care in line with each customer's personal Support plan Being a Key Person for supporting people with health needs, including addiction Developing and maintaining relationships and being comfortable liaising with referral partners (local Mental Health Teams and Adult Social Care in Cornwall Council) Continuously assessing needs and risks appropriately to ensure the service remains suitable for the customers Ensuring caseload management systems are kept up to date Completing and regularly reviewing Risk Assessments Job Details

The Candidate should have: An understanding of support needs and support planning. Experience of risk assessing and risk management. A great communicator with the ability to develop relationships Accurate record keeping and computer skills Confident in lone working. Able to work under pressure and meet deadlines. Knowledge of welfare and housing benefits. A full understanding of safeguarding. SUMMARY: This role is being offered on a full-time, 2-month contract with the possibility of extension and will be field-based in the Cornwall area. This role will be offered with an essential car user allowance, as a car is essential and driving between customers is a large part of the role.
