This is an exciting opportunity to support adults with Motor Neurone Disease (MND), who live in Enfield. As part of service transformation work and investment from the ICB, this post has been created to work across services to provide a link between primary and secondary care, local charities, the voluntary sector, and other key stakeholders in order to support high-quality evidence-based seamless patient care and management.

After several years of ever closer working, North Mid and Royal Free London are planning to come together as one organisation to enable them to go further and faster in improving services for patients and the health of the local community than they can achieve separately. The merger of North Mid into the RFL group is currently expected to take place on 1 January 2025, subject to NHS England and Secretary of State approval.

Main duties of the job

To lead the co-ordination of care and service development for all patients with motor neurone disease within the London Borough of Enfield. To ensure that they and their carers receive quality assured multi-disciplinary care, support, and experience within agreed protocols and allocated resources, at the point of referral to a general neurologist and throughout their journey with the condition. This includes working with and co-ordinating care via a multi-disciplinary MND Care team including colleagues from neurology, respiratory, speech and language, nutrition and dietetics, occupational therapy, physiotherapy, palliative care, social care, and the Motor Neurone Disease Association (MND Association).

North Mid is part of North Central London integrated care system - consisting of the NHS and Local authority organisations in Camden, Islington, Barnet, Enfield and Haringey. As with other ICS's, we are working increasingly closely with partners and indeed many of our financial and performance objectives are measured at this system level. Whilst all organisations remain as standalone, statutory bodies we have an ICS infrastructure for making shared decisions and agreeing shared approaches.

Job description

Job responsibilities

To develop expertise and resources to provide sound clinical advice where necessary and coordinate with the wider multidisciplinary team to provide a holistic and comprehensive approach to patient care for patients with MND. To lead and co-ordinate MND MDT meetings. To provide home visits as well as clinics where patient assessments are carried out to enable appropriate interventions to be implemented, utilising the wider multidisciplinary team. To monitor local provision against relevant best practice guidance, working as a colleague with other MND coordinators and neurological specialists across the London North Central Integrated Care System (ICS).

Person Specification

Education and qualifications

Essential

Registered Nurse or HCPC registration. Relevant specialist post basic programme in an area of neurology or related speciality. Evidence of continuing professional development. Degree in Nursing/ Physiotherapy/N & D /SLT or other related clinical profession, or demonstrable experience at this level.

Desirable

Previous experience of formal/informal teaching of patients & staff.

Skills and abilities

Essential

Presentation skills. Demonstrates a clear vision of the role and service. Demonstrates skilled written and verbal communication skills. Developed clinical assessment skills. Keyboard and word processing skills.

Desirable

Evidence of advanced or enhanced communication skills training.

Experience

Essential

Demonstrates up to date knowledge and expertise in general nursing or physiotherapy or occupational therapy or speech and language or N & D, and parameters for onward referral. Specialist knowledge of working with patients with Motor Neurone disease. Experience of identifying complex needs and appropriate onward referral to optimise function, improve quality of life, and maintain safety within a patient's environment. Experience of working within the community setting.

Desirable

Extensive detailed knowledge of impact of neurological impairment on body systems and processes. Ability to recognise implications of national standards and guidelines and their influence on practice. Experience of identifying measures for specific clinical outcome measurement.

Personal qualities

Essential

Full driving licence and use of a car for travel between locations as appropriate to the patient's needs, linking with key professionals. Committed to multi-professional team working and working with partners. Able to work autonomously and within a team.
