Job summary The post holder will be a Registered Health Care Professional either Nursing or Allied Health Care Professional, with experience and a special interest in Motor Neurone Disease (MND). You will work as a part of a multidisciplinary team with the Rehabilitation Medicine Service within North and North East Lincolnshire, providing specialist support to patients from diagnosis to end of life care, including referral for families for bereavement support where required. You will be expected to work closely with regional MND multidisciplinary teams (MDT) to enhance networks to ensure patients benefit from the most clinically appropriate and expeditious service available regionally or locally and that the needs of carers are also recognised and supported where possible.
