Neuromuscular Care Advisor - East of England

This is an exciting opportunity to join a unique and specialist team in a largely Community based post, suitable for a Therapist (Occupational Therapist / Physiotherapist) or Nurse, working with paediatric and adult patients across specialist and Community Care environments in the East of England, specifically including parts of mid and North Essex, as well as parts of Cambridgeshire. The role supports the care of patients with genetic and acquired neuromuscular disorders such as muscular dystrophy, spinal muscular atrophy and other muscle wasting conditions. You will support the regional specialist Neuromuscular Clinics at Addenbrooke's Hospital in Cambridge as well as Neuromuscular Clinics in other hospitals / clinical settings in the region. Neuromuscular conditions often require complex multidisciplinary care, working in multiple settings including Clinics, Inpatient Units, homes, schools / colleges and other Community settings. You will provide coordination, advice and support for people living with a neuromuscular condition, their families and their carers. The post is part time and makes up a regional service made up of three in a small and very effective team. Main duties of the job
You will work directly and semi-autonomously supporting people living with neuromuscular conditions in the East of England and alongside the specialist Neuromuscular Teams at Addenbrooke's Hospital as well as Community based Paediatric, Neurology and Therapy teams. The regional Neuromuscular Care Advisors are based at Addenbrooke's Hospital, but the nature of the work offers opportunity for highly flexible working, including work from home. Many aspects of the role are now successfully managed using virtual platforms. Each Care Advisor covers a specific geographic area within the region and individuals interested in the role are encouraged to contact the service for further information on how this is currently set up and how caseloads are reviewed and shared out. Qualifications
Registered Paediatric or Adult Nurse (NMC), Occupational Therapist (HCPC) or Physiotherapist (HCPC) Evidence of continuous professional development Experience
Experience of delivering specialist advice and an understanding of the challenges faced relating to identification, assessment, treatment and ongoing management of a long-term condition Relevant post qualification NHS experience in Band 6 or equivalent role Experience of formal / informal teaching Experience of initiating change Experience of working in Primary Care and Community settings Knowledge
Understanding of evidence-based practice Skills
Commitment to multidisciplinary working Additional Requirements
