To clinically assess patients for NHS Continuing Healthcare eligibility in patients' homes, care homes, and hospitals. To follow the process laid out in the National Framework for NHS Continuing Healthcare. Participate in MDT eligibility recommendations. Ensure assessments are robust. Complete new referrals for NHS Continuing Healthcare within 28 days as per the National Framework for Continuing Healthcare. Main Duties of the Job

Carry out joint assessments with Social Service colleagues to identify individuals whose care needs meet the criteria for NHS Continuing Healthcare. Proactively manage a delegated caseload and have the ability to function autonomously. Demonstrate timely forward planning and the ability to organise oneself. Complete relevant specific core reviews/assessments for NHS funded individuals, e.g., Deprivation of Liberty/Liberty Protection Safeguards. Deal with people with complex needs and challenging behaviour, including those approaching end stage of life. Manage difficult family situations and dynamics requiring a high level of competent effort, objectivity, and the use of reflective practice and team support. About Us

Experience

Educated to degree level or equivalent level of experience of working at a similar level Working knowledge of safeguarding adults and the Mental Capacity Act Clear communicator with report writing skills Independent thinker 2 years post qualification experience, including caring for complex patients Community nursing experience, independent sector, or hospital experience Further training evidenced Good understanding of Health and Social Care environments Workforce development knowledge and experience Disclosure and Barring Service Check
