Evaluate equipment, adaptations, and support required for patients to return home safely or transition to another care setting. Liaise with the multi-disciplinary team (MDT), including doctors, nurses, social workers, and physiotherapists, to contribute to holistic discharge planning. Requirements of the Occupational Therapist: Degree or equivalent in Occupational Therapy. Recent UK experience working as an Occupational Therapist. Recent NHS experience. Registered with the HCPC. Community experience is desirable. Experience in discharge planning of patients from hospitals.
