What you will do: Conduct comprehensive assessments and provide evidence-based interventions utilising the occupational therapy process to support older patients. Deliver personalised, goal-orientated rehabilitation programmes aimed at enabling patients to remain at home, reducing care package needs, and delaying care home placements. Actively develop and implement a comprehensive framework of care that reflects a holistic approach to service user needs, aligning with the seven pillars of clinical governance. Prescribe and order adaptive equipment ensuring patients are educated in their safe use. Work collaboratively within a multidisciplinary team to meet the complex needs of service users effectively. Engage in professional development activities to keep skills up-to-date and maintain high care standards. Requirements of the Occupational Therapist: Degree or equivalent in Occupational Therapy Recent UK experience working as an Occupational Therapist Recent NHS experience Registered with the HCPC Living and working in Hounslow offers a unique experience.
