Conduct comprehensive assessments of patients within acute wards, ensuring appropriate discharge planning and recommendations. Collaborate with a multidisciplinary team to develop and implement tailored therapeutic interventions. Monitor and evaluate patient progress, documenting outcomes effectively. Provide education and support to patients and their families to facilitate recovery and independence. Participate in multidisciplinary meetings, contributing valuable insights and expertise.
