Conduct comprehensive assessments using standardised tools and holistic approaches to formulate integrated care plans. Perform detailed falls risk assessments, collaborating with a multidisciplinary team to create effective treatment strategies. Maintain clear and effective communication with all members of the healthcare team to ensure patient care goals are achieved to the highest standard. Provide supportive and informative guidance to patients and their families, promoting health and recovery.
