Collaborate with a multidisciplinary team to ensure seamless patient care and effective treatment strategies. Assist in planning patient discharges, ensuring that all necessary support and resources are in place for a smooth transition from hospital to home or other care environments. Conduct thorough assessments of patient progress and adapt treatment plans as necessary to achieve optimal outcomes. Educate and empower patients and their families, providing them with the knowledge and skills needed to continue rehabilitation outside the clinical setting.
