Job summary As a Specialist Health Care Assistant, your role involves conducting physical health assessments for individuals with identified mental health needs. You will collaborate with various healthcare providers, primary care teams, families, and carers to ensure that the physical health requirements of patients with mental health conditions are addressed effectively. Your responsibilities will include supporting patients in accessing other relevant services, participating in service user reviews, and promoting good health practices. You will also play a crucial role in reaching out to patients who may struggle with engagement and targeting vulnerable and hard-to-reach. Additionally, there may be a component of the role dedicated to conducting physical health checks for patients with eating disorders, particularly those who are stable and engaged in their treatment. Working across Peterborough, Wisbech, Cambridge, and Huntingdonshire, use of own car is a requirement. Working hours: 8-6, 4 days a week with a set day off each week. Main duties of the job Conduct phlebotomy, ECG recording, blood pressure monitoring, pulse and temperature measurement, skin review, and testing of proximal muscle strength. Raise awareness of health promotion, assist with screening services, support long-term condition checks and medication reviews, and identify early signs of relapse or changes in social care needs. Maintain accurate records, adhere to professional standards and ethics, stay updated on relevant guidelines and legislation, participate in training and appraisal processes, and collaborate within a team-based care approach. Ability to work collaboratively in a multi-disciplinary team. Proficiency in record-keeping and database management. Experience of working with Mental Health.
