0.6 FTE. Required for SAS as part of North West Melbourne Aged Care Assessment alliance. Your New Role: Based in North Western Melbourne suburbs, The Single Assessment Service is a dynamic and supportive team that is passionate about delivering high-quality aged care assessments and enabling access to services under My Aged Care (replacing the previous RAS and ACAS system). As a Non Clinical Assessor, you will be responsible for completing non-clinical in-home assessments. You will play a crucial role in enhancing the quality of life for older Australians by conducting in-person assessments and connecting them with essential services. You will work within a multi-disciplinary team and will benefit from a full induction training program to set you up for success to enable better lives for older Australians. Merri Health and its consortium partners (DPV Health, Darebin Council, Merri-bek Council, IPC Health and Your Community Health) have been chosen by the Commonwealth to deliver the new Single Assessment Service in North West Melbourne areas. You Will Be Responsible For: Commitment to ensure that older adults are treated with dignity and respect and an awareness of the impacts of ageism. Complete Home Support Assessments using the Integrated Assessment Tool (IAT) in line with program guidelines, timeframes, and quality requirements using the standardised Aged Care Assessment Quality Framework. Complete the required volume of aged care assessments as per contractual and reporting requirements. In completing the Aged Care Assessment, follow the principles in the My Aged Care Assessment Manual. Conduct reviews for re-assessment as required. Take a Wellness and Reablement approach to Aged Care Assessments. Facilitate clients' progress towards achieving increased stability and improved quality of life by assisting aged clients, according to individual support/service plans, to access relevant commonwealth aged care services and state-based health and community support services, leisure and recreational activities, medical and allied health services. Support the delivery of coordinated and transparent services through the centralised client record and My Aged Care Gateway. Work remotely within a region where you will be required to travel between clients. What You'll Need to Succeed: Experience in an aged care setting preferable. Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach. Experience in conducting client assessments and/or service delivery in a community or health-related position. Demonstrated knowledge and/or experience in working with people who are aging and their families/carers. Demonstrated understanding/experience of the provision of community-based support services to the aged which reflect a wellness approach with a focus on reablement for clients, including assessment, care planning, monitoring, and evaluation processes. Experience/understanding of assessment and goal-oriented support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services. Intermediate skills in Word, Excel, Outlook, and client management systems. Current Australian driver’s licence comprehensively insured and vehicle and the ability to travel within your service area.
