We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations, and well-person check-ups. Practice Nurses are available to help with assessing minor illnesses, minor injuries, dressings, injections, removal of sutures, ear syringing, advice on travel immunisations, health promotion advice, blood pressure checks, smears, etc.
Key roles and functions: Provide a high quality, safe, and supportive environment for patients, meeting their identified physical and psycho-social needs within the scope of individual accountable practice. Work within Practice guidelines, policies, protocols, and procedures, reporting outcomes to appropriate clinical staff so standards of care can be monitored and improved where necessary. Adhere to infection control, equality and diversity, health and safety agendas. Maintain effective communication with patients, carers, relatives, and members of the multidisciplinary team, ensuring information is shared appropriately and confidentially. Protect and maintain patient records to the agreed Practice standard and in accordance with professional record keeping standards. Administer medicines in accordance with a non-medical prescribing framework, patient specific direction, or patient group direction. Organise, manage, and prioritise own workload ensuring care of patients is paramount. Develop the role by using evidence-based practice and continuously improving own knowledge.
Job responsibilities: Chronic disease management: Respiratory Disease (Asthma/COPD), Diabetes, Coronary Heart Disease. Assist in/perform routine tasks related to patient care as directed by senior nursing staff and GPs. Contraception, Cytology, Wound care / ulcer care / Doppler, Travel Health, Removal of Sutures, New Patient Medicals / Urinalysis, ECGs, Venepuncture, Hypertension Management, Ear syringing, Routine immunisations / Childhood immunisations. Assist GPs with minor surgery and coil fittings, chaperoning and assisting patients where appropriate. Request pathology tests, following agreed clinical protocols with referral to senior nurses or GPs as appropriate. Maintain and clean equipment used by the nurses and GPs. Maintain general tidiness and cleanliness of nurses and treatment rooms. Participate in administrative systems in the Practice and maintain accurate records. Attend and participate in any Practice meetings when required. On occasion, undertake home visits. Attend the Lead Nurse meetings for the LS25/LS26 Primary Care Network (PCN) for peer support and sharing best practices.
Assignment and review of work:
The Nurse is responsible to the Lead Nurse for clinical guidance and professional management, work review, and formal performance appraisal. Workload is generally autonomously managed within professional and practice guidelines, policies, and directives.
