Middle Chare Medical Group are recruiting on behalf of Chester Le Street Primary Care Network for a highly motivated, experienced Advanced Nurse Practitioner to join our well-established home visiting service. The role will be to assist in home visits to patients in and around Chester Le Street that are registered in one of the seven local practices. Any allocation of home visits not used will be changed to appointments to help with demand. Main duties of the job

In this role you will work independently, making professional, autonomous decisions. The role will involve initial history taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care. You will be responsible for making any necessary referrals, requests for investigations and do these in an appropriate manner and timescale. The role will involve independent prescribing within the scope of professional practice. The advanced nurse practitioner will support the PCN GP team managing home visits and some same day appointment requests from patients. The nurse practitioner will need to prioritise and triage the needs of patients accordingly, seeking GP input as appropriate where the problem lies outside their professional capability. They will demonstrate safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills. Provide direct access to patients seeking health care management and advice. The successful applicant will be an Independent Nurse Prescriber who can work within the legal framework of nurse prescribing and local prescribing policies. Ability to formulate a diagnosis using medical diagnostic problem solving skills. Undertake a thorough history, physical examination, invasive and non-invasive procedures/investigations. Initiate appropriate diagnostic tests including radiology, imaging and laboratory tests with attention to safety, cost effectiveness, acceptability and efficacy. Work in a collaborative manner with other agencies to ensure seamless service delivery for patients. Assess, diagnose, treat or refer patients as an autonomous primary care practitioner. Participate in the delivery of a range of chronic diseases with an awareness of the Quality and Outcomes framework. Request pathology/radiology investigations and be able to process/manage these results. Apply educational knowledge and skills to support the development of other members of the clinical team. Demonstrate evidence-based practice in line with clinical governance principles and to provide clinically effective care. Provide health promotion advice on an individual basis. Demonstrate an awareness of other community and support resources available and provide necessary information to the patient and carry out required referrals. Demonstrate awareness of cultural or language barriers to health and take action to mitigate these. Be able to conduct consultations using medical interpreter services for those patients who require this support. Ensure own clinical competence and updating through ongoing professional development and the active participation in clinical and staff meetings. Provide professional and clinical support and advice to other members of the primary healthcare team. Keep professionally updated and be responsible for addressing any unmet personal training needs. Experience

Experience of working in a primary care environment. Experience of chronic disease management. Experience of prescribing and undertaking medication reviews. Experience of working as a practice nurse or community nurse. Qualifications

Registered Nurse. Post graduate diploma or degree (Advanced Practice Qualification). Qualified Nurse Prescriber. Qualified Triage Nurse. Teaching Qualification. ALS, PALS. Disclosure and Barring Service Check
