This is a Part-time position reporting to the Executive Director of Greenwood Court in Stratford. The Registered Nurse with Extended Class: RN(EC) privileges (herein referred to as the Nurse Practitioner (NP)) is a management position with the roles of practitioner, consultant, educator, and advocate. The RN(EC) practices autonomously, offering the full scope of primary health care nurse practitioner practice. He/she functions as a member of the interdisciplinary care team in the development, implementation, and evaluation of the resident's plan of care.

The Nurse Practitioner collaborates with the Medical Director, other physicians, and community services and agencies in the provision of primary health care.

The Nurse Practitioner collaborates with the Directors of Care and multi-disciplinary teams in the provision and advancement of nursing services, in the development, implementation, and evaluation of the Nursing department.

Role Responsibilities:

Perform the responsibilities of the position within the legislative and regulatory standards set out in the applicable Provincial Acts. Perform the responsibilities of the position consistent with the Operational policies of Greenwood Court.

Work within the legislated scope of practice of the Nurse Practitioner, including diagnosing, prescribing, ordering, and interpreting lab tests/x-rays/ultrasound as per regulations. Provide direct comprehensive care to residents with specialized and complex needs.

Collaborate and consult with the multidisciplinary team to ensure comprehensive assessment, analysis, treatment planning, and evaluation of resident's care.

Consult with and refer to other health care professionals and community services as needed.

Provide medical coverage on request of the Medical Directors and other physician staff.

As a consultant, assist, collaborate with, guide, and support staff with challenging clinical situations. Provide nursing consultation for clinical policy and practice guideline development and implementation. Collaborate with Management in the development, implementation, and evaluation of clinical policy and practice guidelines.

As an Educator, provide ongoing information to staff, residents, and families, fostering involvement, choice, and autonomy. Contribute to the planning, initiating, implementing, and evaluation of educational programs and learning resources for residents, families, staff, and the community.

As an Advocate, promote evidence-based primary health care services and nursing care of older adults by collaborating with the Medical Directors, medical staff, RNs, and appropriate external agencies. Counsel and support residents and substitute decision-makers and families regarding advanced care planning and consent to treatment issues.

Participate in Management meetings on request, Professional Practice Committees, Pharmacy and Therapeutics Committees, and appropriate Committees as required.

Liaise with community services and agencies to ensure appropriate utilization of specialized services in the Homes.

Protect own health and health of others by adopting safe work practices, reporting unsafe conditions immediately, and attending all relevant in-services regarding occupational health and safety.

Qualifications:

Current registration with the College of Nurses of Ontario as an RN (EC), Nurse Practitioner (unrestricted)

Working knowledge of College of Nurses of Ontario standards of practice

Excellent communication, leadership, and problem-solving skills

Demonstrated current expertise in geriatric care & adult chronic disease management

Proven ability to establish positive relationships and team building

Self-directed, works independently, and is able to achieve outcomes

Valid driver's license and a vehicle required

Reporting Relationships:

This position reports to the Executive Director Greenwood Court.
