Your typical working day would consist of 15 minute face to face appointments, some telephone appointments, reviewing eConsultations and dealing with appropriately, reporting on any tests you have requested, onward referring appropriately, for example patients requiring 2 week wait referrals etc, responding to any 'tasks' sent by team members, assisting with emergency situations, participating in achieving QOF targets, participating in Extended Access when required, participating in practice audits. We are looking for an experienced, dynamic, passionate, motivated, and enthusiastic Nurse Practitioner with a confident 'can do' attitude to join our friendly, growing, patient-focused, supportive practice team. Previous experience in minor illness and triage in a primary care is essential. You must be able to demonstrate safe clinical decision making for patients of all ages acting within professional boundaries. Applicants must have completed their non-medical prescribing and be confident working autonomously in primary care, as well as under the guidance of our GPs. The post holder will be an experienced NP who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the practice. The following are the core responsibilities of a Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Receive and review patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs. This can be face-to-face, by telephone or remote digital triage (eConsult). Manage patients presenting with a range of acute and chronic medical conditions, by assessment, diagnosis, treatment, prescribing and referrals. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the BNF guidelines. Request, process and interpret tests as necessary. Proactively administer vaccinations opportunistically, for example flu vaccination. Act as clinical governance lead for the Practice in areas of special interest and ensure significant events are reported using Practice procedures.
